Probiotics 101 - what to know before choosing a probiotic supplement

The word “probiotic” now appears on everything from supplement bottles and yoghurt tubs to fermented drinks. It is often used as shorthand for good gut health, but that description leaves out an important detail: probiotics are not a single ingredient with a single purpose.
Different probiotic micro-organisms perform different functions, and their effects can depend on the strain, dose and health concern being addressed. Choosing a probiotic supplement therefore requires more than finding the highest number on the front of the pack.
What are probiotics?
Probiotics are live micro-organisms, usually bacteria or yeasts, that may provide a health benefit when consumed in adequate amounts. Many types of micro-organisms already live in and on the human body, with a large and diverse community found in the digestive tract.
Together, these micro-organisms form part of the gut microbiome. They interact with the food we eat, the intestinal lining and the immune system. Some help break down compounds in food, produce useful substances and limit the growth of less desirable microbes.
A probiotic supplement introduces selected live micro-organisms into this environment. Its purpose is not to sterilise or completely repopulate the gut. A suitable strain may support a particular function or help the microbial community recover when it has been affected by illness, medication, dietary changes, stress or travel.
Why does the probiotic strain matter?
Probiotic names can look unnecessarily complicated, but each part provides useful information. A label may list a genus, species and strain, such as Lacticaseibacillus rhamnosus GG. The letters or numbers at the end identify the strain.
One way to understand this is to think of dog breeds. A German Shepherd and a Beagle are both dogs, but they have different characteristics and are suited to different tasks. Probiotic strains can differ in much the same way. Two strains belonging to the same species should not automatically be expected to produce identical results.
A label that lists only “Lactobacillus” or “Bifidobacterium” tells the consumer little. These are broad groups containing numerous species and strains. A well-labelled supplement should identify the micro-organisms in enough detail for their documented uses to be checked.
Evidence for one strain cannot simply be applied to every other probiotic. A useful question to ask is: “Does this strain, at this dose, have evidence for the reason I am considering it?”
What can probiotics help with?
Evidence for probiotics is usually linked to specific digestive concerns rather than broad promises of better health.
Certain strains may help reduce the risk or duration of some forms of diarrhoea, including diarrhoea associated with antibiotic use. Others have been studied for constipation, lactose digestion and symptoms such as abdominal discomfort, bloating or irregular bowel movements.
Probiotics are also widely discussed in relation to irritable bowel syndrome, or IBS. Research results remain mixed, partly because studies often test different strains, combinations and doses. A product that helps one group of people or one IBS symptom may not have the same effect in another setting.
The gut has close links with the immune and nervous systems. A large proportion of the body’s immune activity is associated with the gastrointestinal tract, where immune cells must distinguish between harmless substances and possible threats. Certain probiotic strains may also help support the way the gut and immune system work together, but their benefits depend on the specific strain rather than probiotics as a general category.
Researchers are also studying the gut-brain connection, which describes the two-way communication between the digestive system and the brain. Findings are still developing, and probiotics should not be presented as treatments for anxiety, depression or other mental health conditions unless a healthcare professional recommends their use as part of an appropriate care plan.
How do you choose a probiotic supplement?
Start by considering why you want to take a probiotic. Different probiotic species and strains have been studied for different digestive needs, so a general “gut health” claim does not necessarily tell you whether a product is suitable for your concern.
You may also see a colony-forming unit, or CFU, count on the label. This indicates the approximate number of live micro-organisms in each dose. There is no single ideal CFU count for every digestive concern. What matters is whether the product provides the particular probiotic strains, in an amount shown to be useful for the reason you are taking it.
Check the dosage, storage instructions and expiry date, as probiotics are living micro-organisms that can be affected by heat, moisture and incorrect storage. A pharmacist or healthcare professional can help you interpret the label and choose an appropriate formulation if you are unsure.
Are fermented foods the same as probiotic supplements?
Yoghurt with live cultures, kefir, kimchi and unpasteurised sauerkraut can introduce live microorganisms while contributing nutrients and variety to the diet. They can form part of a gut-supportive eating pattern alongside vegetables, fruit, legumes, whole grains, nuts and seeds.
However, “fermented” does not always mean “probiotic”. Processing or pasteurisation may remove live microbes, and the strains or quantities in a food may not have been tested for a specific health benefit. Fermented foods remain valuable, but they are not necessarily interchangeable with a supplement selected for a defined purpose.
What are prebiotics?
Prebiotics are compounds, often specialised forms of dietary fibre, that are used by beneficial micro-organisms in the gut. They occur naturally in foods such as onions, garlic, leeks, oats, legumes, bananas and certain vegetables.
Probiotics introduce live micro-organisms, while prebiotics help nourish parts of the microbial community already present. Some supplements combine both and are known as synbiotics. As with probiotics, the ingredients and doses should be clearly identified rather than judged by the category name alone.
Can probiotics cause side effects?
Healthy adults generally tolerate probiotics well, although mild bloating, gas or changes in bowel habits can occur when beginning a new supplement. These effects often settle as the digestive system adjusts. Persistent or severe symptoms warrant medical advice.
People who are critically ill, have severely weakened immune systems, use certain medical devices or have serious underlying conditions should consult a doctor before taking probiotics. Parents should also seek professional guidance before giving probiotic supplements to premature or medically vulnerable infants.
A probiotic is best chosen for a clear reason, with attention to the strain, dose, formulation and supporting evidence. The right supplement may offer useful support. The word “probiotic” on its own is only the beginning of the conversation.
